Golden Hammers

I haven't played Smash Bros. in quite a while, and since I've gotten back into it i've been trying to complete the challenge wall.

Two questions:

1. I only have 10 challenges left and have all 5 golden hammers left, but I want to know the EXACT squares and their names to which I can't use a golden hammer on.

2. Does using a golden hammer on a spot that it can't be used on still make it go away?

1. Only the very top left one cannot be hammered. Boss Battles on Intense.

2. No, it'll just make a sudden "clank" noise, and you'll still have all hammers.

Would this be a better question for the 1-Player mode section?

Basically every challenge including boss fights, unless you have the PAL version, there everything can be opened with the hammers. And if you use a hammer on a challenge that can't be opened, then no. It doesn't disappear.
